Mike L., what is that form that the PF is filling out a little after the one minute mark?  Is that like a dispatch?  Is it something most airlines use, and if so, is it possible to get a scan of it?

Are you talking about the "Log Book" the Capt is filling out or the single sheet form the FO is doing?

The logbook.  Is that the A/C log, or is it a flight planning tool?

That is an A/C log book. I actually have one from the B727 I got from Champion Airlines. I'll take some pics for you this weekend...
